histologic aspects of CS, providing initial evidence that it is an aggressive neutrophilic inflammatory process.Childhood maltreatment involves dynamics between the type of maltreatment experienced and the context of maltreatment. Reports of alleged maltreatment to child protective services may overlap and shift over time, complicating understanding of their independent and interacting nature, including how child protection systems respond. Latent class analysis (LCA) and latent transition analysis (LTA) were used to construct data-based models of longitudinal dynamics of alleged maltreatment throughout childhood. We sought to identify patterns leading to system decisions to substantiate allegations of maltreatment and place children in foster care. Using linked birth and child protection records, we defined a cohort of children born in California in 1999, 29.4% of whom had at least one referral for alleged maltreatment before their 18th birthday. Maltreatment and perpetrator indicators were coded, and LCA identified five alleged maltreatment classes and one class of children without referrals. LTA determined consistency of classifications and estimated transitions between classes over age periods. Children with multitype maltreatment patterns or experiences of neglect were most likely to experience future maltreatment allegations. Estimated probabilities of placement indicated children with Multitype Maltreatment allegations were more likely to experience substantiated maltreatment allegations and out-of-home placements. Findings identify a repeatable method for better understanding complex systems.This study investigates the regulatory effect of plasmacytoid dendritic cells (pDC)/myeloid dendritic cells (mDC) imbalance on balance of Th1/Th2 and Th17/Treg in primary immune thrombocytopenia (ITP). A total of 30 untreated ITP patients and 20 healthy controls were recruited. Compared with healthy control, the pDC proportion of ITP patients was significantly reduced (P = 0.004), while the mDC proportion was not significantly changed (P = 0.681), resulting in a decrease in the pDC/mDC ratio (P = 0.001). Additionally, compared with controls, serum levels of interleukin (IL)-6, IL-12, and IL-23 were increased in ITP patients (P  less then  0.001), and mRNA levels of IL-12p40, IL-12p35, and IL-23p19 were also increased (P =0.014, P = 0.043, P  less then  0.001). Compared with the healthy control, the proportion of Th1 and Th17 cells in ITP patients increased (P = 0.001, P = 0.031). Serum levels of interferon gamma (IFN-γ) and IL-17 in ITP patients also increased (P = 0.025, P = 0.005). Furthermore, T-bet and RO443, P = 0.014; r = -0.471, P = 0.011). The imbalance of pDC/mDC and the increase of IL-6, IL-12, and IL-23 lead to the increased differentiation of CD4+ T cells into Th1 and Th17 cells, which might be the important mechanisms underlying the imbalance of Th1/Th2 and Th17/Treg in ITP patients.Nursing records are an account of patient condition and treatment during their hospital stay. In this study, we developed a system that can automatically analyze nursing records to predict the occurrence of diseases and incidents (e.g., falls). Text vectorization was performed for nursing records and compared with past case data on aspiration pneumonia, to develop an onset prediction system. Nursing records for a patient group that developed aspiration pneumonia during hospitalization and a non-onset control group were randomly assigned to definitive diagnostic (for learning), preliminary survey, and test datasets. The final verification used the test data and revealed the highest compatibility to predict the onset of aspiration pneumonia (sensitivity = 90.9%, specificity = 60.3%) with the parameter values of size = 80 (number of dimensions of the sentence vector), window = 13 (number of words before and after the learned word), and min_count = 2 (threshold of wordcount for word to be included). This method represents the foundation for a discovery/warning system using machine-based automated monitoring to predict the onset of diseases and prevent adverse incidents such as falls.By August 2020, non-Hawaiian Pacific Islanders-4% of Hawaii's population-accounted for 30% of the cumulative COVID-19 cases in the state. Micronesians, mostly Chuukese and Marshallese, were the most severely affected. Disproportionate COVID-19 infection in racial or ethnic groups in the US occur because of socioeconomic factors. The COVID-19 pandemic can be thought of as a syndemic-where cases cluster "on a background of social and economic disparity". In this brief report, we describe factors that put Chuukese and Marshallese at increased risk for COVID-19 in Hawaii. We show that Micronesians had increased risk for COVID-19 due to limited employment opportunities, housing insecurity, and underlying comorbid conditions in the context of rescinded federal health insurance and broken government promises. We also highlight the resiliency that many community members demonstrated in preventing new infections and supporting those infected. We conclude that COVID-19 in Hawaii should be understood as a syndemic, where Micronesians were disproportionately affected due to disparities in housing, employment, and health access. Our work supports efforts to continue addressing underlying socioeconomic disparities in creating a more equitable future for our Micronesian community in Hawaii.
Paroxysmal localized hyperhidrosis is a rare disorder of the central autonomic nervous system. No association between paroxysmal hyperhidrosis and severe headache has been previously described in literature.
A 65-year-old woman with idiopathic paroxysmal localized hyperhidrosis combined with severe holocranial headache attacks is described in this case report. Extensive diagnostic testing by means of laboratory examinations, 24-hour urinalyses, chest X-ray, abdominal ultrasound and computed tomography scans, and brain and spinal cord magnetic resonance imaging could not identify an underlying disorder. A diagnosis of idiopathic paroxysmal localized hyperhidrosis was made, and the patient was successfully treated with clonidine 0.075 mg three times a day, without any side effects.
Paroxysmal localized hyperhidrosis is a rare central autonomic nervous system disorder that can occur in combination with severe headache. Both the headache and paroxysmal hyperhidrosis complaints were treated effectively with clonidine in the patient described in this case-report.
