Paediatric testicular and para-testicular lesions have traditionally been managed according to adult protocols. Testis-sparing surgery (TSS) has gained popularity as it has become apparent benign lesions predominate in childhood. Frozen-section examination (FSE) for intra-operative diagnosis has been extensively utilised in adults, though its use in paediatric practice remains limited. We reviewed our experience of FSE in paediatric patients with an aim to identify the utility and efficacy of this tool in the management of testicular and para-testicular pathology.
A retrospective, single-centre review of paediatric patients who underwent intra-operative FSE for a range of testicular and para-testicular lesions was performed. FSE results were compared to final pathology. TSS was performed if appropriate, and was utilised in adolescent patients, and in lesions with a diameter greater than 20mm.
Nine males underwent FSE from 2013 to 2020. Median age at surgery was 9years (range 1-15). Eight (89%) patients had benign pathology. FSE result correlated with the final pathological examination in 100% of cases. FSE facilitated TSS in 7/9 cases.
FSE has 100% diagnostic accuracy for paediatric testicular and para-testicular pathology. We would recommend all lesions be evaluated by FSE to guide intra-operative decision making and facilitate TSS in appropriate cases.

The relationship between hospital volume and outcomes remains unclear in the delivery of burn care in resource-limited settings, where demand often exceeds capacity. We sought to characterize the association between burn patient volume and the use of operative intervention at a tertiary burn unit in Malawi.
This study examined patients admitted to Kamuzu Central Hospital located in Lilongwe, Malawi, over years 2011-2019. We described the association between the census at the time of admission and the use of operative intervention, as well as the time to operation. Patient census was defined as low (≤ 15 patients), medium (16-29 patients), and high (≥ 30 patients).
A total of 2484 patients were included. The mean daily burn unit census was 22.5 patients (SD 6.6) and varied significantly by season. For the medium and high census, the adjusted risk ratio of undergoing surgery was 0.79 (95% CI 0.64, 0.97) and 0.65 (95% CI 0.49, 0.85), respectively, adjusted for flame burn, age, %TBSA, and delayed presentation. At a low admission census, the adjusted mean time to operation was 17.2days (95% CI 14.4, 20.1) compared to 28.3days (95% CI 25.4, 31.2) at a high census.
In a resource-limited setting, an increasing mean daily census significantly reduced the use of operative intervention and increased time to operation, potentially increasing burn-associated morbidity. In order to improve the quality of burn care in similar environments, improved resource allocation during busier seasons and targeted burn prevention efforts are imperative.

Early decompressive posterior linear craniectomy (PLC) can be indicated in very young infants with complex multisuture synostosis, which often involve the lambdoid suture (LS). The literature data on the surgical technique and its results are scarce.
Based on our experience with PLC during the last 10 years, we detail our surgical technique for PLC, the possible pitfalls, and complication avoidance.
We review seven observations, 5 girls and 2 boys, 6 of these with identified mutations, operated for PLC at a mean age of 3.19 months (6 days to 6.1 months). One patient died of unexplained cardiac arrest on postoperative day two, the others had a favorable outcome with good development and no visual loss. Three of these required additional cranioplasty at a later age, one of these with craniofacial distraction.
PLC can be a salvage operation in very young patients with complex synostosis involving the LS, and with proper preparation and careful technique, allows favorable outcome. The approach must be versatile in order to anticipate further surgeries in these complex, most often syndromic cases.

The acetabulum has been reported as a reliable age estimation marker. However, analyzing its morphological changes can be challenging using computed tomography (CT) imaging. Newly introduced global illumination rendering (GIR) applied to CT can improve the visualization of the fine details and thus the method's performance. This study aimed to analyze age estimation using morphological features of the acetabulum using GIR applied to CT.
We collected 200 postmortem CT scans. A segmentation of the acetabular joint was initially done. Then, three-dimensional (3D) reconstruction of the images was performed using GIR. These images were saved and then analyzed by two operators based on the three morphological criteria described in the Rougé-Maillart method. Reproducibility was assessed by intraclass correlation (ICC). Age estimation was assessed by multiple linear regression.
