The aim The aim of the research was to analyze the results of observation andexamination of COPD patients in order to identify a group of individualswith potentialasthma overlap.
Materials and methodsWe have conducted a two-stagedynamic investigation of 43 COPD patients during 3-8 years. The patients were divided into two groups group 1 counted 30 individuals who presented with at least one episode of reversible bronchial obstruction (RBO) during the observation; group 2 - 13 individuals who presented with nonreversible bronchial obstruction (nonRBO). Results It was revealed that around 70% of COPD patients have occasional episodes of RBO. It was established that the level of blood eosinophils in these patients on the whole is rather low even in people with intermittent RBO, and the total IgE level appeared to be significantly higher in patients with intermittent RBO comparing to the level of this marker in patients who have nonRBO.
ConclusionsCOPD patients with intermittent RBO and high level of total IgE level form a group with potential asthma overlap.

The aim of the study is to study the features of the clinical phenomenology of depressive disorders at the present stage, taking into account gender and age factors.
Materials and methods Clinically examined 107 men and 138 women with bipolar affective disorder (current episode of depression), depressive episode and recurrent depressive disorder using HDRS, HARS, BDI, C. Spilberger's Scale of Reactive and Personality Anxiety.
Results In all patients showed an increase in the severity of depression and anxiety with age, but in women the greatest differences were found between the middle and older age groups, and in men when comparing young and middle age groups. In young patients the level of agitated depression is higher for men, and women showed the higher overall level of anxiety, somatic anxiety, undifferentiated depression, reactive and personality anxiety. In patients aged 30-44, men have a significantly higher overall score of HARS depression, the level of adynamic, agitated and undifferentiated depression, the level of depression on BDI. In the older age group, the levels of depression in men and women are not significantly different, and the levels of mental anxiety and personality anxiety in women are significantly higher.
Conclusions The general tendency consisting in increasing the severity of depression with age, and more pronounced indicators of depression in men, and anxiety in women. In men, the greatest increase in the severity of depression occurs before the age of 44 years, and in women after 45 years.

The aim to compare the efficacy of pectoral nerve block type II and thoracic paravertebral block for analgesia during and after breast cancer surgery.
Materials and methods Sixty adult women were undergoing unilateral radical mastectomy or quadrantectomy with axillary dissection. Patients were randomized to receive either pectoral nerve block with ropivacaine 0.375% 30 ml or thoracic paravertebral block with ropivacaine 0.5% 20 ml. Evaluated variables included pain intensity at 0, 2, 4, 6, 12, 18 and 24 hours, intraoperative fentanyl, 24-hour postoperative opioid (promedol) and nonopioid (ketoprofen) consumption, the time to first rescue analgesia.
Results There were no statistically significant differences between pectoral block and paravertebral block groups in intraoperative fentanyl consumption 2,2 (1,81-2,81) vs 1,9 (1,63-2,25) mcg/kg/hour (Р>0,05) and in the pain intensity during the first 24 hours after operation. The mean postoperative 24-hour promedol and ketoprofen consumption was 4,0 (±8,14) mgvs 5,0 (±8,85) mg (Р>0,05) and 66,7 (±66,09) mgvs 95,8 (±90,78) mg (Р>0,05) in the pectoral and paravertebral block groups respectively. Time to the first analgesia request was longer in pectoral block group - 540 (455,0-600,0) min vs 515 (265,0-650,0) min (Р>0,05). There were no complications after pectoral blocks and 2 complications after paravertebral blocks.
Conclusions in breast cancer surgery pectoral nerve blocktype II can provide postoperative analgesia comparable to thoracic paravertebral block with lower complications rate.

The aim Improvement of early diagnostics quality and efficiency of chronic pharyngeal diseases (CPD) treatment in children with DM-1 by studying the clinical and laboratory features of the disease course and complex therapy administration.
Materials and methods A comprehensive examination of 707 children with diabetes mellitus-1, aged 3 to 18, was conducted, which determined clinical and laboratory features of their state in the presence and absence of chronic tonsillitis (CT).
Results Using microbiological and cytological examination of the palatine tonsils (PT) crypts, the leading role of microbial associations in the pathogenesis of HT in the formation of immune inflammation was determined, their influence on the local immunity indices was defined. Significant disturbances of the microcirculatory bed (MCB) in the presence of HT in children with DM-1, direct correlation between CPD and the presence of vascular glomeruli, changes in the retinal capillary structure (r = 0,332; p <0,05) according to biomicroscopy bulbar conjunctiva (BMBC) data were determined. The study of psycho-emotional state in children with CT and DM-1 was conducted, along with applied psycho-corrective program, which significantly reduces the manifestations of aggressiveness, aggression, and frustration reactions in them.
Сonclusions A non-invasive complex of the study of PT state on changes of local immunity indices, microbiocenosis and cytological data for correction of CPD course was developed, which allows to obtain faster compensation of CT and contributes to patients' quality of life improvment.
