Stress and burnout have been soaring among doctors. It does not only have deleterious effects during working hours but also impact personal lives of the doctors. The primary focus of this study is to gauge level of burnout among surgical residents working in two major tertiary care hospital of Karachi, Pakistan.
This was a cross sectional study comprising of 118 candidates who completed the questionnaire based on demographics variables, professional details and Maslach Burnout Inventory (MBI), which scores the burnout level on the basis of three components namely emotional exhaustion, depersonalization and personal accomplishment. The duration of study was one month starting from 1st January to 31st January 2019. Residents working in Department of General Surgery were part of inclusion criteria. Consultants and medical students met the exclusion criteria. Data was analysed by SPSS-23.
Mean MBI score was 57.15 with mean scores of Emotional Fatigue, Personal Fulfilment and Depersonalization were 22.42, 19ng for extensive hours and felt financial constraints, there is an extensive need of comprehensive support groups, humane number of working hours, improved salary packages, de-stressing activities like sports etc. to ameliorate the mental health of resident physicians and enhance their productivity.

This standard based audit was conducted at an undergraduate and postgraduate teaching hospital. One thousand and sixty four intra oral periapical radiographs were graded according to the set standards by NRPB by 3 examiners. Data were analyzed with SPSS-24.
Out of the 532 conventional periapical radiographs, 313 radiographs were Grade 1, 177 radiographs were Grade 2 and 42 radiographs were Grade 3, requiring a repeat radiograph. For 532 digitally taken periapical radiographs, 255 radiographs were Grade 1, 192 radiographs were Grade 2 and 85 radiographs were Grade 3 and diagnostically unacceptable. Considering the aforementioned results, the findings of the first cycle did not meet the standards.
According to the results of the quality assurance audit, the radiographs were below the standards set by NRPB. Recommendations were made for improvement measures in the radiology department and plan to re-audit after six months.

This study was conducted to see the frequency of hormone receptors and Her-2/Neu positivity in different histology in breast cancer patients.
It was a cross-sectional study conducted at the Department of Medical Oncology, Jinnah Postgraduate Medical Centre from June 2018- March 2019. Total 373 patients of age 22-81 years of which 360 female and 13 males, with histopathological proven diagnosis of breast cancer were included in the study using non-probability consecutive sampling technique. The immunohistochemistry (IHC) was performed on biopsy sample for the status of PR, ER and HER-2/Neu and the confirmation of Her-2/Neu was done by Fluorescent In situ Hybridization (FISH) technique if HER-2/Neu was equivocal by IHC testing. Information regarding demographics, family history, histology, grade, stage, metastatic site and other histopathological parameters were noted on predesigned proforma by the researcher. SPSS-23 was used to analyse data.
Total of 373 patients were included in the study. The mean patient age was 45.34±12.28 years. ER, PR, Her 2 Neu (IHC) & Her 2 Neu (FISH) were found positive in 206 (55.2%), 182 (48.8%), 121 (32.4%) & 23 (6.2%) patients, respectively. The histology with PR receptor & grade of tumour with ER & PR receptor showed statistical significance (p<0.05).
ER, PR & HER-2/NEU expressions in breast cancer vary among different population and it is very important to find out the frequency among different histopathological types as it is of predictive and prognostic value. So, it is recommended to look for these markers and treat them accordingly.

Metabolic Syndrome is strongly associated with Rheumatoid Arthritis, which significantly increases cardiovascular complications and hence morbidity and mortality. Treating Metabolic Syndrome decreases cardiovascular related disease flares and deaths. This study evaluates prevalence of metabolic syndrome in young Rheumatoid Arthritis patients.
This Cross-sectional study was conducted in rheumatology department of a tertiary care hospital in Karachi. All diagnosed cases of rheumatoid arthritis from April to August 2018 were consecutively included. Disease activity of rheumatoid arthritis assessed by Clinical Disease Activity Index (CDAI). Associate determinants of rheumatoid arthritis were measured along with outcome variables.
Out of 104 rheumatoid arthritis patients, 34 (32.7%) found to have metabolic syndrome in whom 20 (58.8%) of patients were seropositive. A significant association of metabolic syndrome was found with age (p-value 0.023), BMI (p-value 0.006), waist circumference (p-value 0.002), FBS (p-value <0.001), SBP (p-value <0.001), DBP (p-value <0.001), TG (p-value <0.001), HDL (p-value 0.022), and Methotrexate drug history (p-value 0.030).
We conclude that metabolic syndrome is highly prevalent in young rheumatoid patients with rheumatoid arthritis. Histone Methyltransferase inhibitor Treating Metabolic Syndrome decreases cardiovascular related disease flares and deaths. This young population base study will help out to estimate the exact burden of Metabolic Syndrome to decrease overall morbidity and mortality.
