Clinical experience suggests that gastroesophageal reflux disease (GERD) occurs commonly in infants with congenital muscular torticollis (CMT). However, this is an understudied topic and prospective studies are absent. We determine the prevalence of GERD in infants with CMT, comparing clinical characteristics between CMT infants with and without GERD, and identifying infants with potentially undiagnosed GERD.
A prospective cohort study of 155 infants with CMT younger than 12 months with and without GERD was evaluated by pediatric physical therapists.
GERD prevalence was 30.3%, including 6 (3.9%) infants with undiagnosed GERD. Demographic and clinical characteristics were similar in CMT infants with and without GERD.
This is the first prospective cohort study determining the prevalence of GERD in infants referred for evaluation of CMT. Further prospective studies are needed to determine whether early intervention and treatment of GERD improves outcomes in infants with CMT (see Supplemental Digital Content 1, available at http//links.lww.com/PPT/A369).

A systematic review evaluated exercise parameters and ages that produced the most improvement in bone among individuals with cerebral palsy (CP) ages 3 to 21 years.
PubMed, Scopus, Ebscohost, and Web of Science identified potential articles. Covidence was used to identify eligible citations and assess bias. The osteogenic index (OI) was used to evaluate intervention parameters.
The database search identified 312 citations. Twelve full-text articles were included. A 1-hour calisthenic exercise program performed 2 to 3 times a week for 8 months targeting each body region had the highest effect size and a substantial OI. Most of the interventions reviewed had low OIs. Activities of longer duration and greater intensity had greater OIs and prepubertal age-enhanced treatment effects.

In recent years, the management of complex lesions in patients undergoing percutaneous coronary intervention (PCI) constitutes a field of high interest and concern for the interventional cardiology. Therapeutic drug monitoring and treatment optimization of clozapine are recommended, owing to its narrow therapeutic range and pharmacokinetic (PK) variability. This study aims to assess the clinical applicability of published population PK models by testing their predictive performance in an external data set and to determine the effectiveness of Bayesian forecasting (BF) for clozapine treatment optimization.
Available models of clozapine were identified, and their predictive performance was determined using an external data set (53 patients, 151 samples). The median prediction error (PE) and median absolute PE were used to assess bias and inaccuracy. The potential factors influencing model predictability were also investigated. The final concentration was reestimated for all patients using covariates or previously observed concentrations.
The 7 included models presented limited predictive performance. Only 1 model met the acceptability criteria (median PE ≤ ±20% and median absolute PE ≤30%). There wasrom the perspective of model-informed prediction dosing, model predictability should be improved using updating or metamodeling methods. Moreover, BF substantially improved model predictability and could be used for clozapine treatment optimization.
Recent cross-sectional studies have identified differences in autobiographical memory (AM) among individuals with chronic pain, but the temporal relationship between the 2 is unknown. Moreover, AM has yet to be studied in patients undergoing major surgery. This study addressed these gaps by conducting a prospective, longitudinal study of memory performance, postsurgical pain, and psychosocial factors in 97 adult participants scheduled for major surgery. Memories were evaluated using the Autobiographical Memory Test before and one month after surgery when participants were asked to recall personal events related to positive and pain-related word cues. Responses were coded for level of specificity, emotional valence, and surgery-related content. Questionnaires assessing presence/absence of pain and psychological functioning were administered before and at 1-, 3-, 6-, and 12-month follow-ups. Generalized estimating equations modelled pain at each postsurgical time point with memory variables as predictors. As 95% CI [1.31-5.37])) and those who produced more surgery-related content at the one-month assessment (OR = 1.31, 95% CI [1.02-1.68]) had greater odds of reporting postsurgical pain up to 12 months later. These findings indicate that presurgical AM biases are risk factors for development and maintenance of postsurgical pain. To the extent that these biases are causal, presurgical interventions that modify the quality and content of patients' memories may prove to be promising strategies in the prevention of chronic postsurgical pain.
Retinal racemose hemangioma (RRH) is a rare, non-hereditary arteriovenous malformation characterized by the appearance of dilated and tortuous retinal vessels. RRH can develop complications associated with retinal ischemia, such as vitreous hemorrhage, retinal vein occlusion and neovascular glaucoma. Here, we report a case of RRH with retinal hypoperfusion detected by wide-field swept source optical coherence tomography angiography (OCTA), which was not unambiguously illustrated by fluorescein angiography (FA).
Case report.
A 57-year-old woman was referred to our hospital for the evaluation of severe retinal vascular tortuosity, dilation and retinal hemorrhages in the left eye. Fundus examination revealed arteriovenous communications temporal to the fovea and multiple microaneurysms surrounded by retinal hemorrhages at the midperipheral temporal fundus. In FA, multiple hyperfluorescent lesions with leakage corresponding to microaneurysms were observed in the temporal and lower midperipheral areas; however, non-perfused areas were apparently absent. By contrast, wide-field OCTA clearly showed low-density retinal capillaries in the superotemporal quadrant in comparison with those in the inferotemporal quadrant.

The objective of this study was to assess effects of SYDCP participation on youth and adults from a rural and urban underserved high school community. We used a mixed-methods approach. Patient-Reported Outcomes Measurement Information System (PROMIS) measures for Pediatric Sense of Meaning and Purpose were measured in high school students. PROMIS Adult Global Health and Self-Efficacy was measured in coached adults. Paired t tests compared pre- and postintervention and 6-month follow-up scores. Thematic analysis was used to analyze focus group discussion of adults. Twenty-five students participated, 15 students coached adults with diabetes or prediabetes. Students' sense of meaning and purpose significantly improved postintervention compared to preintervention. Diet and physical activity behaviors improved. Adolescent-adult relationships mediated participation benefits. Our study showed SYDCP improved adolescents' sense of meaning and purpose. In addition, youth and adult relatedness led to improved health behaviors. These findings have important implications, as a sense of purpose and youth-adult connectedness are associated with health behaviors and psychological well-being. Further larger studies of health education programs that engage related youth-adult dyads and assess long-term behaviors and health outcomes are needed.
To report a unique case of PPS maculopathy with remarkable rapid progression over 2 years. These findings show the importance of early detection of macular disease in order to limit toxic exposure and reduce the risk of progression.
Multimodal retinal imaging including fundus autofluorescence (FAF), near infrared reflectance (NIR) with pseudocolor, and spectral domain optical coherence tomography (SD-OCT) were performed in an elderly patient with a history of PPS therapy (cumulative dose of 1205 grams) at baseline and 2 years later.
Baseline multimodal retinal imaging failed to show significant macular findings of PPS toxicity in either eye but upon repeat evaluation 2 years later advanced features of PPS maculopathy were detected in both eyes with FAF, NIR, pseudocolor, and SD-OCT.
This report describes a remarkable case of rapid progression of PPS maculopathy as documented with multimodal retinal imaging. The dramatic progression of macular findings over just 2 years underscores the importance of early detection and prompt withdrawal of therapy, if systemically feasible, to retard the development and rate of progression of PPS maculopathy.
