In September 2014, the Government of India introduced legislation requiring all tobacco packaging to display a health warning covering 85% (60% pictorial, 25% text) of the principal display area of the pack, of at least 4cm in height and 3.5cm width, with legible text in a white font on a black background and in English and / or the same Indian language as the language used on the pack. We evaluated compliance with this legislation in the Udupi district of Karnataka, India.
We procured one example of every tobacco pack of cigarettes, beedis, chewing tobacco and snuff sold by a convenience sample of retailers in one urban and two rural areas in each of the five administrative blocks of the Udupi district between June to August 2018. For each pack we measured the size of the health warning, calculated the proportion of the pack covered, and assessed the legibility.
A total of 365 packs were collected from retailers, with 357 of these branded and eight, all packs of snuff, unbranded. Warnings on 320 (87.3%gs on tobacco packaging are an important and inexpensive means of communicating the harms associated with tobacco use. However, relatively few studies have explored regulatory compliance with warnings. We collected all unique tobacco packs from 66 retailers in a district in Karnataka in India and assessed whether each met the legislative requirements in terms of warning size and proportion of the pack covered, legibility, and the language used. Of the 365 packs collected, only one in eight was compliant with the legislative requirements. This study highlights the importance of assessing warning compliance and the need for enforcement in India.We investigated associations of quantitative levels of N-glycans with haemoglobin A1c (HbA1c), renal function and renal function decline in type 1 diabetes. We measured 46 total N-glycan peaks (GPs) on 1565 serum samples from the Scottish Diabetes Research Network Type 1 Bioresource Study (SDRNT1BIO) and a pool of healthy donors. Quantitation of absolute abundance of each GP used 2AB-labelled mannose-3 as standard. We studied cross-sectional associations of GPs and derived measures with HbA1c, albumin/creatinine ratio (ACR) and estimated glomerular filtration rate (eGFR), and prospective associations with incident albuminuria and final eGFR. All GPs were 1.4 to 3.2 times more abundant in SDRTN1BIO than in the healthy samples. Absolute levels of all GPs were slightly higher with higher HbA1c, with strongest associations for triantennary trigalactosylated disialylated, triantennary trigalactosylated trisialylated structures with core or outer arm fucose, and tetraantennary tetragalactosylated trisialylated glycans. Most GPs showed increased abundance with worsening ACR. Lower eGFR was associated with higher absolute GP level, most significantly with biantennary digalactosylated disialylated glycans with and without bisect, triantennary trigalactosylated trisialylated glycans with and without outer arm fucose, and core fucosylated biantennary monogalactosylated monosialylated glycans. Elevated HbA1c is associated with an altered N-glycan profile in type 1 diabetes. Although we could not establish GPs to be prognostic of future renal function decline independently of HbA1c, further studies to evaluate their impact in the pathogenesis of diabetic kidney disease are warranted.The complexities of antibiotic resistance mean that successful stewardship must consider both the effectiveness of a given antibiotic and the spectrum of that therapy to minimize imposing further selective pressure. To meet this challenge, we propose the Desirability of Outcome Ranking approach for the Management of Antimicrobial Therapy (DOOR MAT), a flexible quantitative framework that evaluates the desirability of antibiotic selection.Herein, we describe the steps required to implement DOOR MAT and present examples to illustrate how the desirability of treatment selection can be evaluated using resistance information. While treatments and the scoring of treatment selections must be adapted to specific clinical settings, the principle of DOOR MAT remains constant the most desirable antibiotic choice effectively treats the patient while exerting minimal pressure on future resistance.
Many inflammatory bowel disease (IBD) patients in remission have persisting symptoms, compatible with irritable bowel syndrome (IBS-type symptoms). We aimed to compare the effectiveness of gut-directed hypnotherapy versus standard medical treatment (SMT) for IBS-type symptoms in IBD patients.
In this multicenter, randomized, controlled, open label trial, patients aged 12-65 years with IBD in clinical remission (global assessment) and biochemical remission (fecal calprotectin ≤100 µg/g, or ≤200 µg/g without inflammation at endoscopy) with IBS according to Rome III criteria, were randomized to hypnotherapy or SMT. Primary outcome was the proportion with ≥50% reduction on a visual analogue scale for symptom severity, as measured with the Irritable Bowel Syndrome Severity Scoring System (IBS-SSS) at week 40 (i.e. 6 months after finishing the intervention), compared to baseline. Secondary outcomes included total IBS-SSS score, quality of life, adequate relief, IBS-related cognitions and depression and anxiety scores.
